CONSENT TO USE ELECTRONIC SIGNATURES AND ELECTRONIC RECORDS

By signing this Consent to Use Electronic Signatures and Electronic Records (“E-Sign Consent”), I hereby consent to electronically conduct this and other transactions with Hinge Health, Inc. (“Hinge Health”).  I understand that this consent applies to all of my electronic transactions with Hinge Health related to Hinge Health’s clinical research.

I understand that I have the right at any time to withdraw my consent to electronically conduct transactions with Hinge Health by writing to Hinge Health, 455 Market Street, Suite 700, San Francisco, CA 94105, attn.: Mindy Hong.  However, I understand that if I withdraw my consent to electronically conduct transactions with Hinge Health, I may no longer be able to participate in the clinical research studies.

I agree to provide Hinge Health with my e-mail address and other applicable electronic contact information.  I understand that if my e-mail address and/or other applicable electronic contact information changes, I can update that information by contacting Hinge Health in writing at the address listed in the previous paragraph or by contacting research@hingehealth.com.   

I understand that I have a right to print and retain a copy of all authorizations contained within this form for my own records, and that I will need to have a computer with internet access to do so.  

This E-Sign Consent complies with the Electronic Signatures in Global and National Commerce (E-Sign) Act, 15 U.S.C. § 7001 et. seq. and the Uniform Electronic Transactions Act (UETA).
