The HingeSelect effect: More high-value options, less unnecessary care

People often don’t know what care they need or how to access it, leading to worsening conditions and climbing costs. Delivering the best musculoskeletal (MSK) clinical outcomes requires support for the full range of care from digital physical therapy (PT) to surgery.

We built HingeSelect to help members find the right level of care earlier, typically starting with preventative options and only escalating when clinically necessary. We’ve expanded our network now to more than 5,000 provider locations, putting 91% of members within the network’s range and the results are clear.

Routing members to the right care

Our hybrid, connected care experience lets us triage members precisely from the start and step up support as needed. Given clear options and education on the right clinical path, 86% of members chose digital PT. Of the rest, 9% needed in-person care and 5% needed virtual specialist visits. This optionality delivers the right care at the right time, and steers most people away from expensive escalation from the outset.

The HingeSelect effect engagement and outcomes

Reducing unnecessary escalation

HingeSelect addresses escalation at two critical points: before a care plan exists, and after an unnecessary intervention has already been suggested. Without early guidance, people often end up pursuing costlier services like surgery simply because they don't know their options.

For most members, recommending PT or other non-operative care options offers an immediate, actionable path forward. Others have already been pointed toward surgery, injections, or imaging before they engage with Hinge Health. For this group, an expert clinical review can redirect to conservative care when appropriate.

Members who have questions about PT or already have a recommendation for an intervention like surgery can start with a video visit with a board-certified orthopedic specialist who can guide them in the optimal direction. This visit includes evaluating their condition, walking through their options, and building a personalized plan. Before these visits, 49% of members had no care plan and nearly 20% expected they would need surgery, injections, or imaging. Afterward, 88% were directed to conservative care like PT, reducing care escalations by 33%.

One member, Falanda, had experienced worsening pain for 5 years and received a recommendation for a cortisone shot that left her feeling defeated. Because her employer provided access to HingeSelect, she connected with a Hinge Health orthopedic specialist who both diagnosed her bulging disk and built her a hybrid care plan – all without having to start over or navigate next steps alone.

Over 90% of in-person requests we field are for PT, proof that HingeSelect drives conservative care as the first step. And when a member sees a provider, over 80% see one from the HingeSelect network. If a member doesn’t have a HingeSelect provider nearby or prefers their carrier network, we help them find a high-quality provider in their network instead so nobody hits a dead end. Closing that loop in a notoriously opaque system has earned our coordinated care a 4.5/5 member satisfaction score.

Why it matters

Our fragmented healthcare system makes it difficult for people to get the MSK care they need, and most have no help navigating it.

Combining digital and in-person care makes conservative treatment more accessible, helping members get started, stay engaged, and avoid unnecessary escalation. When higher-acuity services like surgery are appropriate, our care team and curated in-person network of surgeons supports members throughout their care journey.

Since launch, HingeSelect's hybrid model has delivered higher-value care, better outcomes, and real savings. Stay tuned for what's next.
